I know I'm missing something simple but at my age...

4 mono outputs
Use Mixer Preset 2 (because 1 is all stereo): Primary 11, 12, 13 sent to the mono outputs, all sent to Intermediate mix 1 for woofer

Bus Groups for Mixer Presets 2 (again for mono): Set Groups 11,12,13 to Mixers 11,12,13 (no indication that mixers are in Mixer Preset 1 or 2) "Number of ranks routed to group" shows correct numbers.

Rank Routing shows 1024 options, all with bus counts=0. Where are the Preset 2 Bus Groups?

Have you selected preset 2 (pg. 208) for the organ?

You have to set that organ's mixer preset to 2 first. On the Audio settings control panel, a button found on Rank routing or under View | Large blah blah.

I never would have seen that setting.
The manual's examples are all headed "Stereo" so I did not read them word by word.

In Hauptwerk v7+, on both the "General settings | Audio mixer" and "General settings | Audio mixer bus groups" screens the entries in the left-hand object lists are organised within expandable nodes according to their mixer presets. The groups for any given preset number (e.g. 2) define groups of that same preset number's buses. Hence the buses within groups within preset 2's node are inherently preset 2's buses.

(As others have mentioned you need to select the desired preset for the organ in order to route that organ's ranks to that preset's groups [and thus to that preset's buses that you've selected within those groups].)
